13 years old need to come with her/his guardian if wan't to have piercing. With over 14 years old we can confirm permission by calling to guardian. Paper notes are useless, because it can be faked too easily.
Every piercing customer is required to fill form before piercing. You need to prove all the information in the form true with your signature.
For some piercing age limit is higher than 13 years.
